Making a Referral
Thank you for your interest in our Federally Qualified Health Centers (FQHC). We welcome new patient referrals.
There are nine FQHCs that are members of the Three Rivers Alliance of Community Health Centers that operate 48 locations across western Pennsylvania. Cost of services are based on an individual’s ability to pay using a sliding scale fee structure.
To make a referral, visit the Our Health Centers page. Most of our centers have multiple locations that can be found by clicking on the individual FQHC site on this page. Phone numbers are provided for each location along with services provided.
Services provided vary based on the FQHC site and include medical, dental, behavioral health and substance-abuse disorders. Access to specialty medical services, lab services, pharmacy support and care coordination also are provided.
